22 Health Members Now Have Access to Approximately 12,000 Caregivers at Cleveland Clinic’s Hospitals and Outpatient Centers
Cleveland Clinic has announced that it is joining the provider network of 22 Health, a new health marketplace division of Community Care Plan serving residents of Broward County, effective Jan. 1, 2026. Under the terms of the new agreement, 22 Health members will have access to approximately 12,000 additional caregivers across the five Cleveland Clinic hospitals and 40 outpatient centers throughout Florida.

22 Health members will have access to care at Cleveland Clinic Weston Hospital, Cleveland Clinic Indian River Hospital, Cleveland Clinic Martin North and South Hospitals, and Cleveland Clinic Tradition Hospital, as well as the numerous outpatient centers located throughout South Florida and the Treasure Coast.

22 Health is a division of Community Care Plan, a health plan with 25 years of experience serving the community and is jointly owned by Broward Health and Memorial Healthcare System. Alongside these two health systems, adding Cleveland Clinic to the 22 Health provider network expands healthcare access for residents of Broward County.
The division will offer Bronze, Silver, and Gold health plans, serving individuals and families through the health insurance marketplace. 22 Health members will benefit from open access plans with personalized, community-based care provided by Cleveland Clinic in Florida, as well as retail and mail-ready pharmacy services, multilingual support, and specialized programs such as case management and chronic disease management.
Open enrollment for 22 Health is currently running through Jan. 15, 2026.
